I don't often comment here, who could possibly have better answers based completely on experience ,than Fred or John. But I had a situation that was similar , we had a bank in the Florida keys ,flooded with salt water 2 days after Hurricane Wilma, the floor had been previously crystallized, long story short, wherever sunshine got to the stone and heated up,salt came blowing out severely spalling the stone creating a similar condition.after trying several different poly and epoxy fills. We finally found great success in using latacrete spectralock epoxy grout mixed with only 50 % of thr filler to make it more flowing, We then spread the entire effected area After 100 grit, let set up on the stone overnight, ground off with the 200 and then proceeded thru the diamond restoration,it bonded broken stones, filled and bonded all the spalling, easily ground perfectly flat, and was easy to make near exact color match, and has worn like iron, much better than the stone
Maybe an option ?? hope this helps   Scott Wilson  Wilson Stoneworks Inc

I agree with John. The stone seems to be crumbling. It could be an issue with the stone itself or the substrate.

Adam,

This damage does not appear to be from the improper filling of the holes it
appears the stone is coming apart and the stone is flaking away from the
fills. I would be careful on this project. If you fill the hole the same
could happen to your fill.

Have you checked for moisture in this stone?
