I have seen this many times with poor quality ceramic tile. It is basically moisture that is getting under the installation and migrating up through the tile. There is not a lot  you can do other than to possibly seal the grout, if the moisture is originating from the top and not from underneath.

A hotel management company that I work for had me inspect their ceramic tile flooring. They say that no matter how they clean it dark spots return within a couple of days. Weather is not a factor, happens throughout the year. I believe that some thing in the setting is bleeding through. I have attached pics and you will notice that in #039, there is a shadow of a trowel line. this also appears on several other tiles. The other pics demonstrate the darkness that reappears every time. the install is about 5 years old in downtown Denver. I am going to go back with a moisture meter and have a tile in my shop which I am trying to replicate their experience with. Any suggestions would be helpful.
