Not sure if this is correct, I've seen a job like this once and ask a tile installer... if it was to be fixed how would you go about it.

 "He said he would remove at least half the sanded grout and fill it with unsanded grout mixed with latex instead of water. He thought that would strengthen the grout enough to hold in place".

I think Fred has the best answer, But I've heard epoxy grout install is pretty challenging.
