I agree with Randy, you need to get a moisture reading
Especially if it's on grade.
I have seen a lot worse when moisture is involved .
If you need a good moisture meter I can send you a picture of the one we use.
If it isnt moisture it may be just improperly applied sealer which can be removed and cleaned.
We use sparks southwest and Richard James products 
For terracotta and Saltillo 
A good combo is stone glamour as a penetrating sealer and then TK6 low gloss topical to finish.
Comes out great.
There are several things you need to know when using 2 products.
Very important, after applying multiple coats
Of stone glamour so it forms a film
On the surface. The. You let it cure for a day or two.
The. Use a hogs hair pad dry to buff the surface so it will take the TK6.
The results are excellent.

I've done several of these jobs, with significantly varying results. Generally speaking, beyond the condition of the sealer, if the tiles look to be in poor shape with lots of discoloration, RUN. Trust me...I've learned the hard way.

If the tiles are in decent condition, but the sealer is simply failing, the job is salvageable. The sealer could be failing due to normal wear and tear or a poor application. What I've found is that if the previous sealer is not completely stripped off, a new sealer application will not bond properly and will fail, similar to your picture. In situations where I've had positive results, I've used products from Aldon.
