Hard to see from the picture but it looks like a symptoms of butt-joint intall and the grout is sitting in the tile bevel and there is not enough space between tiles for expansion so when temperature and moisture changes stone edges pressing together and edge is blistering off. i think there is also a little of pinkish color around which means moisture. This limestone is very soft to begin with so it crumbles like a cookie.

3. is there moisture present?
4. How is the tile set?
5. Is it slab on grade?
