is the spalling only on the edges?  how extensive is it? is it all over or just in certain areas

yes I have seen this lots of times.  as I tell all my students to check the
following and get back to me

1. is the tile hollow?
2. is the tile loose?
3. is there moisture present?
4. How is the tile set?
5. Is it slab on grade?
