Looks like a butt joined tile to me as well.
The issues are probably occurring where the joints are less than 1/16 ".
Mostly when I see this the joints are all different sizes.
You can change out tiles
If they have extra and widen the joint before grouting.
You need to be able to push the grout down to the base of the tile.

1. is the tile hollow?
2. is the tile loose?
3. is there moisture present?
4. How is the tile set?
5. Is it slab on grade?
