Like John said that’s rust and it is pointing right to the culprit. On the floor that little tiny diamond triangle piece of stone if you look to the right there is your problem for the rust spots on the floor and on the wall. The grout is missing and one little spot and that’s all it takes.
I would check all the areas on the wall for the same issue this run your finger nail down the grout lines it will tell you if it’s solid or not. If the girl comes off with your fingernails there’s your problem.
If the customer has spare tiles I would take that one out of there and make up brand new piece that is able to cover the whole area.
